SMART-HF is an online AI app for risk stratification of heart failure patients, based on the Japanese heart failure registry (JROADHF) of just over ten thousand patients. SMART-HF predicts the prognosis of a one-year survival ratio after hospitalization due to acute decompensated heart failure. This app was designed not only for healthcare providers but also for non-healthcare providers.

The predictive performance of SMART-HF was confirmed to be equal or superior to the conventional risk models with internal validation. External validation results will be provided following future research.
